Therapeutic Whole-Lung Lavage for Pulmonary Alveolar Proteinosis: A Procedural Update.
نویسندگان
چکیده
Pulmonary alveolar proteinosis is a disease caused by increased accumulation and impaired clearance of surfactant by alveolar macrophages. This narrative review summarizes the role of therapeutic whole-lung lavage in the management of pulmonary alveolar proteinosis. We describe the preprocedural evaluation, indications, and anesthetic considerations, along with step-by step technical aspects of the procedure, postoperative recovery, potential complications, and long-term outcomes.

Pulmonary alveolar proteinosis and first successful whole lung lavage in Sri Lanka: a case report
BACKGROUND Pulmonary alveolar proteinosis is a rare disease characterized by accumulation of lipoproteinaceous material within alveoli. There are three clinically distinct forms: congenital, acquired and secondary. Whole lung lavage is currently the gold standard therapy for severe cases of pulmonary alveolar proteinosis. In Sri Lanka this is the first reported successful whole lung lavage for ...
متن کاملAlveolar proteinosis in extremis: a critical case treated with whole lung lavage without extracorporeal membrane oxygenation.
Pulmonary alveolar proteinosis is a rare idiopathic lung disease characterized by the accumulation of lipoproteinaceous material within the alveoli, which impairs gas transfer and decreases the ventilation/perfusion ratio, and can lead to respiratory failure. Whole lung lavage is the most effective therapy for pulmonary alveolar proteinosis, but may not be tolerated by patients with severe resp...
متن کاملAnaesthesia for serial whole-lung lavage in a patient with severe pulmonary alveolar proteinosis: a case report
INTRODUCTION Pulmonary alveolar proteinosis is a rare condition that requires treatment by whole-lung lavage. We report a case of severe pulmonary alveolar proteinosis and discuss a safe and effective strategy for the anaesthetic management of patients undergoing this complex procedure. CASE PRESENTATION A 34-year-old Caucasian man was diagnosed with severe pulmonary alveolar proteinosis. He ...
متن کاملSubglottic stenosis following repeated double-lumen tube intubation for whole-lung lavage in a teenager with pulmonary alveolar proteinosis
Pulmonary alveolar proteinosis is a rare disorder with decreased clearance of surfactant from the pulmonary alveoli resulting in progressive respiratory distress. Whole-lung lavage remains an important procedure for symptomatic relief in patients with pulmonary alveolar proteinosis. If a double-lumen tube can be placed, lavage of one side of the lung is performed through one lumen while the pat...
متن کاملAnesthesia for lung lavage in pediatric patient with pulmonary alveolar proteinosis.
BACKGROUND AND OBJECTIVES Pulmonary Alveolar Proteinosis (PAP) is a rare disorder first described in 1958. The Whole-Lung Lavage (WLL) proposed in the 1960s, remains the treatment of choice. Several techniques have been described to perform lung lavage in pediatric patients; however, all have limitation and risks. CASE REPORT Female patient, aged 6 years and 8 months, 25 kg, diagnosed with pu...
